1074 Gallstone Ileus: Two Decades After Cholecystectomy

Abstract Introduction Gallstone ileus is a rare complication of gallstones that occurs in 1%-4% all cases bowel obstruction. We present surprising case gallstone causing small obstruction 19 years after open cholecystectomy. Case Report A 77-year-old male presented with 3-day history abdominal pain, 4 episodes vomiting and absolute constipation. He had surgical background an cholecystectomy appendicectomy 45 ago respectively. Medically, he well-controlled hypertension experienced TIA 5 prior. Computed Tomography Scan the abdomen pelvis revealed features consistent obstructing, heterogenous opacity distal without pneumobilia. The patient subsequently underwent diagnostic laparoscopy. Intraoperatively, obstructing gallstone, measuring cm, was found 50cm proximal to ileocaecal junction, dilatation collapse. Enterotomy removal stone done. Post-operatively, this gentleman recovered complications discharged home two days later being able tolerate solid diet. Conclusions This second reported previous about decades ago, according our literature search. Although extremely rare, absence gallbladder does not exclude possibility ileus.
